Assisted reproductive technology (ART)

Assisted reproductive technology

Under the assisted reproductive technologies means:

  • Intrauterine insemination with sperm of her husband
  • In Vitro Fertilization
  • In vitro fertilization with micromanipulation ICSI
  • The use of donor sperm in ART

The introduction of sperm directly into the uterine cavity provides an artificial passage of cervical sperm, which could be lost in the cervical factor infertility when trying to conceive naturally.

Indications for intrauterine insemination are also ejaculatory dysfunction, immunological infertility, oligoastenozoospermiya. Immunological factor infertility can be overcome by pre-treatment sperm. In the case of unknown cause of infertility is also possible in vitro fertilization using intrauterine insemination with semen quality satisfactory performance (number of motile sperm for more than 5 million).

Insemination of donor sperm to avoid direct sexual intercourse.

The technique of in vitro fertilization, involves stimulation of superovulation, which can cause a number of complications, such as direct stimulation, so as a result of multiple pregnancies. The success of the procedure of stimulation to a large extent depends on the woman's endocrine system. It is known that pregnancy rate is higher in those cases where puncture of follicles can be aspirated to 5 eggs. Next, oocytes were aspirated by transvaginal ovarian puncture. After this, fertilization occurs in vitro - the exposure of eggs in a medium with processed sperm. This is followed by transfer of embryos into the uterus and the reduction of the embryos.

Correction of azoospermia remained minimal until the introduction of methods of injection of sperm into the cytoplasm of a mature oocyte (ICSI), which requires only a single insemination for sperm epididymal or testicular origin.

The indications for IVF (ICSI) is:
  • sperm concentration below 10 million (2 million for ICSI)
  • motility 2 - less than 10% (5% for ICSI)
  • more than 70% pathological sperm forms (over 96 for ICSI)
  • only the possibility of obtaining sperm surgically
  • failure of other treatments.

The effectiveness of artificial insemination is around 30% in one trial. The use of assisted reproductive technology significantly enhances the correction of the most severe forms of male infertility, including azoospermia.

Proper and effective treatment of infertile marriage is possible only if the implementation of a clear interaction between urologists, gynecologists, and specialists of IVF. It is mandatory that the patient fully informed about all possible methods for restoring fertility indicating the percentage of successful events and risk of complications in himself and his wife.
